Team Captain Registration

Fill out the form below to register yourself as a team captain, after you have made payment for your team, you will be able to add additional players and fill out your team's roster. A team can have between 3-5 players and the registration fee is $125.00.

Adult and Big Mac Division teams can choose their level of play. Teams are seeded based on height and the age of your oldest team member.

Before registering and participating in this event, every player and his/her guardian (if the player is under 18) must read and execute a Waiver form. There are risks connected with my participation in this tournament and its related activities. I know that participating in strenuous activities such as basketball is potentially hazardous. I assume all risks associated with playing in this event including, but not limited to, falls, contact with other participants, effects of the weather, and the condition of the playing surface. I release, waive, discharge and covenant not to sue Special Olympics Oregon and/or Trail Blazers Street Jam, event sponsors, event charities, and their workers, employees, volunteers, agents and directors, from any and all action, suits, demands and claims of whatever nature in law or in equity, from any injuries suffered by me while participating in this event or its related activities and further from the loss or damage to personal property by theft, negligence or otherwise. Players' eligibility for NCAA, collegiate sports and local school districts vary. The event organizers are not responsible for determining each player's eligibility.
